Nagle et al. (2017) identified that nursing relies on data-based research to improve care outcomes, safety, and quality of care delivered. Today, data has become increasingly indispensable in making informed decisions; hence nurse professionals must be in awe of diverse data in their line of duty and how to apply them to make effective and informed decisions. Data availability allows nurses to identify a pattern regarding patient response and outcomes. With increasing focus on improving overall care delivery, one approach has been to enhance the quality of care through reduced errors, reduced readmission rate, and improved care quality. To reduce readmission rates, nurses utilize diverse forms of data to ensure the best care is accorded to patients before and after discharge Application Of Data To Problem-Solving Essay Discussion.
A sample scenario is when healthcare has been having a high rate of patient readmission among Heart failure elderly patients. To improve the care experience and outcomes, nurses and other care providers are supposed to collect data that indicates the trend in patient discharge and readmission. The data that could be used in this case include the number of heart failure discarded within a given period, the number of patients from the discharged group readmitted within 30 days of discharge, and the patient’s complaints regarding their health status.
Based on the provided scenario, the data trend obtained can be utilized to inform the stakeholders, i.e., nurses, clinicians, leaders, and overall care-providing team, about the factors leading to a high patient readmission rate after discharge from a heart failure treatment. The obtained provides critical information and knowledge, including causes of readmission and possible intervention measures to address the issue. A high readmission rate in a healthcare organization indicates poor care quality, ineffective pre-discharge planning, and ineffective follow-up process by diverse stakeholders.
Clinical reasoning and evidence-based judgment give a nurse leader an edge regarding decision making, hence positively impacting care delivery practices, improved performance, safety and quality of care practices, and overall address to patient needs to their satisfaction (Sweeney, 2017)Application Of Data To Problem-Solving Essay Discussion. A nurse leader in today’s healthcare setting relies on clinical reasoning and judgment through the collected data about care delivery and patient conditions before adopting and implementing a specific treatment plan. Regarding the scenario, the nurse leader would adopt the data indicators to identify the probable causes behind the high turnover rate among the released Heart failure patients. The leader would develop pre-discharge interventions to effectively ensure the patient transition from hospital to home care is smooth, minimizing possible readmission in the future. More so, the after-discharge care or follow-up practices would change to accommodate more updates and close monitoring of the patient through collaborative care between hospital and family-or home-based care providers.
The different forms of data available to the nurse leader would be critical in informing intervention measures or problem-solving practices while improving on nursing care knowledge to be adopted in the future regarding addressing the high readmission rate. According to McGonigle & Mastrian (2022)Application Of Data To Problem-Solving Essay Discussion, data has been a critical foundation for better organization and new developments. The more information/data a nurse leader has about a case or patient, the more optimal efforts can be adapted to deliver the best probable outcomes. Through nursing informatics tools, nurse leaders and healthcare professionals can immediately provide more patient-aligned and proper care, improving the healthcare sector.

DESCRIPTION:
Respond to at least two of your colleagues* on two different days, asking questions to help clarify the scenario and application of data, or offering additional/alternative ideas for the application of nursing informatics principles.
POST ONE
Nursing informatics involves a collaborative knowledge of nursing, computer, and information science to aid in data collection, information, knowledge, and wisdom in the nursing practice (McGonigle & Mastrian, 2022). Nurses use this every day without realizing it. Every time we see a patient, we do a nursing assessment which is nursing science; we then use computer science to input the assessment into the patient’s chart. Information science is how information is organized, managed, stored, and used. This occurs in our assessment as we gather and store patient data (University of Michigan, 2023). When nurses use nursing informatics, they can build a proper foundation, collect relative information, and refine knowledge as needed while producing new knowledge. This allows nursing to care for their patients properly. Knowledge modeling is a crucial step in building knowledge. This model helps nurses understand the processes of knowledge modeling and the techniques involved in developing knowledge (Yun et al., 2021). The knowledge model involves knowledge acquisition, dissemination, generation, and processing (McGonigle & Mastrian, 2022)Application Of Data To Problem-Solving Essay Discussion.
While working in wound care, my patients benefited greatly from nursing informatics and the knowledge model. Our office constantly researched and tested new products to ensure we had the best and most up-to-date products and information. A marketer would bring us products, but we would not take what they said at face value. We did our research. We would take these products and choose a few patients to try them on. If the patient agreed, we would place the product on them and track them over a few weeks. If the product showed promise, we would keep it, but if it did not aid in the healing process or time, we would remove it and let the company know our thoughts. By doing this, we healed our patients promptly and helped these companies fill knowledge gaps to improve their products.

POST TWO
Improving Medication Safety through Data Analysis
Nurses are a vital part of the healthcare system, and they have long been responsible for administering, managing and delivering care to their patients. In today’s world, computer science plays an increasingly important role in the healthcare industry, and nurses can now take advantage of a variety of powerful tools to help them acquire and manipulate data and information (McGonigue & Mastrian, 2022)Application Of Data To Problem-Solving Essay Discussion.
Medication errors are frequently encountered at our facility as an enormous problem affecting patient safety. These errors occur throughout various stages, from prescription, administration, and documentation. To address this issue, a hypothetical scenario is created to demonstrate how access to and application of data can facilitate problem-solving and knowledge formation to improve medication safety.
Little johns et al. (2020) assert that data might be collected from various sources to improve medication safety. Electronic drug administration records (eMARs), pharmacy data, incident reports, and patient feedback are a few instances of these sources. These data sources could serve as a useful tool to find out more about patient reactions along with prescription orders, doses, and administration timings. An integrated health information system might provide access to this data, enabling nurses and other medical personnel to acquire and analyze the required data safely and effectively (Tijhuis et al., 2019)Application Of Data To Problem-Solving Essay Discussion.
Insights into the root causes and recurring patterns of medication errors may be gleaned by analyzing the gathered data. Medical professionals can identify opportunities for improvement by looking at the many types of errors, how frequently they occur, and the factors that contribute to them. The information gained from this data may be utilized to create evidence-based interventions and strategies to prevent medication errors, like putting barcode scanning systems in place, educating staff about medication safety, and fostering better collaboration and communication among members of the healthcare team (Dinov, 2016)Application Of Data To Problem-Solving Essay Discussion.
According to Sommers (2018), nurse leaders play a crucial role in utilizing clinical reasoning and judgment to form knowledge from this experience. They would analyze the data, identifying trends, patterns, and risk factors associated with medication errors. Nurse leaders would collaborate with multidisciplinary teams, including pharmacists, physicians, and information technology specialists, to develop targeted interventions based on the data findings. They would also consider the feasibility, cost-effectiveness, and potential barriers to implementation while formulating strategies for improving medication safety. Additionally, nurse leaders would use their clinical expertise and knowledge of best practices to guide decision-making and ensure that interventions align with regulatory guidelines and standards.
To conclude, the hypothetical scenario of increasing medicine safety through data analysis underlines the vitality of having access to and using data in the healthcare industry. Medical professionals can uncover medication error patterns, provide evidence-based treatments, and enhance patient safety by collecting and analyzing data from diverse sources. Using their clinical expertise and judgment, nurse leaders play an instrumental role in using data to generate knowledge and promote organizational change to enhance drug safety protocols Application Of Data To Problem-Solving Essay Discussion.
