As deaths due to cancer are declining, this number is projected to increase to 21.7 million by 2029 (National Cancer Institute Division of Cancer Control and Population Sciences Office of Cancer Survivorship, 2019). As cancer death rates continue to decline, more and more Americans are living with the long-term effects of cancer treatment, giving rise to cancer survivorship care. Although cancer survivorship care has become the golden standard of care, it has not yet made its way into the curriculum of APP education in many Nurse Practitioner and Physician’s Assistant programs (Halpbern et al., 2016). Although the Sunshine Comprehensive Cancer Center opened a cancer survivorship clinic, APPs in the Thoracic Surgery Cancer Clinic have not been making referrals nor writing cancer survivorship care plans for patients. Part of the challenge is this is not included in their educational preparation as APPs. The team wants to take an evidence-based quality improvement (EBQI) approach to address this issue so they can confirm best practice and effectively , but they are nervous as to whether they trust their evidence-based practice (EBP) skills enough to engage in the EBP process without an EBP mentor. After creating a PICOT, and searching, appraising, and synthesizing the evidence, it was apparent that cancer survivorship care plans (SCPs) and referring patients to the cancer survivorship clinic are best practices. Although the structures and processes are already in place, providers have not been educated about cancer survivorship. The team has chosen to utilize the Model for Improvement (Institute for Health Improvement, 2017) to develop, test, implement, and spread changes that would result in creation of SCPs and referrals to the cancer survivorship clinic.
