PICOT Question Example 1

My clinically relevant research question in PICOt formula is: In elderly patients with dementia, are the effects of providing spiritual care compared to not providing spiritual care beneficial to patient outcomes? I do not really have a time frame as I did not really see any time reference in my search. My question is based on therapy as opposed to diagnosis, prevention, or etiology. (LHSPeoria, 2016). I arrived at this topic because of my recent job as a Clinical Care Coordinator for elderly patients with the California MediCal Waiver Program. 

My job is to help patients who can no longer live alone, be able to stay in as much of a home-like environment as possible and try to keep them out of skilled nursing facilities. This saves both the government and the patient money and adds a sense of greater independence in the elderly population. Through my work I frequently encounter patients with dementia, and I often feel helpless and without resources and words. 

I know I need to do my assessment and the patient often brings up concerns and problems that have no clear solution and I feel bad for them. There are some patients who are very open about their spirituality and like to have prayer or speak to clergy.  Other patients with dementia are often not able to verbalize their needs and so this area is grossly neglected.  It will be interesting to see what effects spirituality has, if any, on patient outcomes for this population group.

When doing a short search thus far, things like fostering personhood, helping develop stress strategies, improving recovery from illness, and developing long-term social interactions were some of the benefits listed. The pursuit and expression of meaning and purpose, as well as a sense of closeness to the present, oneself, others, the natural world, and the significant or sacred, are all considered aspects of spirituality. (Britt et al., 2023).

Rationale:
I arrived at this topic and question due to the significant clinical implications of fluid management in dialysis patients. Fluid overload contributes to hypertension and cardiovascular complications, which are leading causes of morbidity and mortality in this population. Effective fluid management protocols could potentially improve outcomes and reduce hospitalizations among dialysis patients, addressing a critical area in nephrology care.

Importance:
This research question is important because optimal fluid management remains challenging in dialysis patients, and there is a need for evidence-based protocols to guide clinical practice. By evaluating the impact of a structured protocol on blood pressure control and cardiovascular outcomes, healthcare providers can potentially enhance patient care and quality of life in this vulnerable population.
