Chronic illnesses, diseases and comorbidities are among the modern health problems which are worsening in prevalence so they have been selected as the national health issue to be investigated. A notable trend is that more individuals are coping with assorted chronic diseases and this challenges the existing healthcare systems across the world. The role of environmental issues in the hospital where I practice can both change and protect patients’ health through different paths. Furthermore, the implementation of a plan to cope with three or more chronic conditions simultaneously can have a tangible effect on patient readmissions and lengthier hospitalization frequencies. Hospitals have seen their resources, for example personnel and available beds, compromised in the process.
In addition, there should be a shift in the care strategies for people with chronic diseases because the high number of disease cases warrants a holistic and coordinated approach in the management of patients (Amalberti et al., 2019). The health care providers might need to augment the operations of their departments to meet the various demands of people who suffer from many chronic diseases by becoming more integrated into different specialties.
Socio-Economic status, health literacy and access to healthcare facilities are the social determinants upholding and potentially managing this health situation. The individuals who have fled their poverty-stricken roots can find themselves in deeper water as they struggle with the difficulties of persistent disease treatment. The obstacles, like lack of access to medical facilities, limited funds to buy medications, and low understanding of disease management techniques (Dixon-Woods et al., 2012)Review Of Current Healthcare Issues Assignment Discussion, place them at a very high risk.
Such improvements in my working system in the health sector include additional ways for combating chronic illnesses occasioned by healthcare problems. To illustrate, the undertaking of care coordination has been centering around the organization of multi-disciplinary care teams and the use of electronic health record technologies that facilitate knowledge sharing between different doctors. Moreover, value has been placed on patient education and empowerment with programs specifically designed to train those who struggle with chronic illnesses to become more knowledgeable and better off. Asides from this, some measures have been put in place to strengthen the preventative care services and build strong community-based healthcare providers that can help lower the prevalence of chronic diseases (Carter & Busby, 2023). The changes demonstrate an attempt to adopt holistic, patient-centred care paradigm for better equipped 21^{st} century healthcare to deal with the ever-growing level of chronic conditions.
