The most potential diagnosis for this client is bacterial vaginosis (BV). Female adults with BV report various symptoms, including a thin gray or white vaginal discharge; a burning feeling with urination; itching, pain, or burning in the genital areas, including outside the vagina; and a strong fish-like odor, particularly after sexual intercourse (Reid, 2018). The client reported vaginal discharge accompanied by a fishy smell, qualifying for bacterial vaginosis as the most appropriate diagnosis. However, a diagnostic test should be ordered to confirm the suspected diagnosis before developing a treatment plan. Specifically, the clinician should order two tests: a wet mount and a vaginal pH test. A wet mount will be used to check the presence of bacteria or white blood cells (WBC) in the vaginal discharge sample. The presence of fuzzy borders in these cells portrays a bacterial infection. A vaginal pH test will also be ordered to assess acidity versus alkalinity in the vaginal discharge sample. According to Lin et al. (2021), bacterial vaginosis increases the pH of the vaginal fluid above the normal range (4–4.5). Thus, a pH above 5.4 denotes bacterial vaginosis.
