Healthcare Program/Policy Evaluation Affordable Care Act or Obamacare or Patient Protection and Affordable Care Act

Description The policy was ratified by the United States federal government in 2010. Further, it is a holistic healthcare reform with several primary objectives (Assistant Secretary for Public Affairs, 2022). For instance, it strives to make health insurance accessible and modest, decrease healthcare spending, and enhance the efficiency and quality of healthcare. This policy comprises several provisions that impact various facets of the American healthcare system like developing the health insurance marketplaces, enlarging Medicaid suitability, enforcing a personal mandate forbidding bias founded on pre-existing conditions, and executing several regulations and changes. Notably, the policy has had notable influences on the United States population and its healthcare system.

How was the success of the program or policy measured?

It was measured via several benchmarks like the number and attributes of individuals who signed up for health insurance plans via exchanges. Also, it was measured via the performance of insurers and health institutions under value-based buying.

Remarkably, the metrics were not all-inclusive or similar and stakeholders possessed diverse outlooks on its success.

How many people were reached by the program or policy selected? How much of an impact was realized with the program or policy selected?

According to the Assistant Secretary for Public Affairs. (2022a), the aggregate sign-up for Medicaid expansion, Basic Health Program, and Marketplace coverage in participating regions in early 2022 reached an all-time high of 35 million individuals. These conclusions and numbers reckon on data from CMS that portrayed another all-time high of 21 million individuals in over 40 states obtained coverage due to Medicaid expansion.

This policy has had a positive impact since the number or rate of uninsured individuals in the last quarter of 2021 was almost at an all-time low of 8.8% (Assistant Secretary for Public Affairs, 2022a)Assessing Healthcare Program Assignment Paper.

At what point in program implementation was the program or policy evaluation conducted?

Some assessments were done before full execution. For instance, the Commonwealth Fund documented a report in 2016 assessed the CARE Act, a submission to repeal the ACA, and juxtaposed its impacts to Obamacare’s forecasted impacts in 2022. Also, the Assistant Secretary for Planning and Evaluation had a brief book published in 2022 summarizing significant findings from various reports on the policy’s achievements between 2021-2022 (ASPE, 2022).
What data was used to conduct the program or policy evaluation?

Data used emanated from the American Community Survey, the Current Population Survey Annual Social and Economic Supplement, the National Health Interview Survey, and the Medicaid and CHIP Enrollment Reports.

What specific information on unintended consequences were identified?

Hospital and organization monopolies. ACA wanted the creation of accountable care facilities, instead, mergers and acquisitions happened, developing monopolies that could increase costs and decrease competition. Also, the Cadillac tax was another unintended outcome.
What stakeholders were identified in the evaluation of the program or policy? Who would benefit most from the results and reporting of the program or policy evaluation? Be specific and provide examples.

State and federal governments – would benefit by gaining intuitions into the equity and success of ACA

Health insurers are other stakeholders – would benefit by comprehending the effect of the policy on their profitability and market share.

The public – would benefit through understanding the costs and gains of the policy and its effect on their health.

Did the program or policy meet the original intent and objectives? Why or why not?

Yes. Firstly, it has decreased the number of uninsured individuals exponentially. Also, it has enabled the provision of critical health protections and benefits for individuals with pre-existing illnesses.

Would you recommend implementing this program or policy in your place of work? Why or why not?

Yes. One reason is that it has given a lifeline to people with pre-existing conditions and also, it has amplified uninsured Americans, mainly those from low-income areas.

Identify at least two ways that you, as a nurse advocate, could become involved in evaluating a program or policy after one year of implementation.

One way would be through conducting interviews, surveys, and case studies with policy stakeholders, participants, and beneficiaries to gather information and suggestions on the policy issues, effects, and results.

Also, I would offer continuous education, development, and growth to participants and my nursing peers and facilitate the execution of quality enhancement enterprises.

General Notes/Comments The ACA wanted to ensure that every American has access to insurance services. To some extent, the policy has accomplished some of its set goals in its fourteen years of existence. Even though there were instances when a section of legislators wanted the Act repealed, it has gone on to reduce the number of uninsured Americans exponentially.
