Anxiety is a type of mental disorder or condition, making it challenging to have a quality day as one feels panic and fear. In the United States, Anxiety has affected an estimated 3.1% of the population (Byers et al., 2018). Women are twice as likely to suffer from anxiety than men, and the condition often occurs together with major depression. It is the most common mental disorder, affecting all age groups. However, the positive thing related to the disorder is that it can be managed via diverse interventions, including pharmacological and non-pharmacological. The proper diagnosis, and pharmacological prescriptions have been utilized to manage the disorder and related symptoms. This paper aims to provide a detailed assessment and treatment prescription for a patient identified with anxiety based on a provided case study.
Decision Option #1
Case summary: the patient is identified as a Middle-Aged Caucasian Man With Anxiety. After examination, the prescribed options entail medications to address the issue with three proposed medication procedures. The first step in managing or treating the patient is prescribing Zoloft 50mg PO daily. According to Bandelow et al. (2022), sertraline (Zoloft) is recognized as a serotonin inhibitor (SSRI), which works in the human body by raising the levels of serotonin which is a critical chemical in the human brain and is involved with regulating one’s anxiety and mood. Based on a report by SSRIs are approved by FDA as a first-measure option for anxiety solutions for adults (Byers et al., 2018)Case Study For Anxiety Disorder Assignment Paper. This was further supported by Strawn et al. (2018) stating that clients who use Zoloft medication were more likely to have dealt with anxiety with the right and appropriate treatment decision.

The decision to use imipramine 25mg PO BID was turned down due to the antihistamine properties experienced when the drug is used for treating GAD. The antihistamine and anticholinergic factors result in unwanted side effects such as dizziness, vomiting, weakness and tingly. Another option that would have been used is buspirone 10mg PO BID. It was less effective in addressing anxiety symptoms as it took longer to manage anxiety and related symptoms among adults. However, deciding to administer the Zoloft medication involved diverse ethical considerations, including autonomy, which empowered the patient to choose which medication best suited his needs (Benecke et al., 2020)Case Study For Anxiety Disorder Assignment Paper. The ethical considerations allowed a balance between administering the best or most effective prescription and respecting patient needs. The treatment process was enacted while measuring the symptoms on the HAM-A, which progressed to the next treatment option. The HAM-A score was critical to establish whether the patient was recovering and responding positively to the selected medication.
Decision # 2
Decision two concerning the client’s treatment would entail continuing with the Zoloft prescription but with an increased dosage from 50 to 75mg orally daily. The increase in the dosage resulted from a patient report of partial response to the previous dosage. However, before the revision of the dosage, the patient’s symptoms were recorded where he had no breath shortness or a tight chest, hence a positive change from the previous record symptoms. On the other hand, it is critical to note the patient recorded a decrease in work-based worries, with his HAM-A score dropping significantly. According to Strawn et al. (2018)Case Study For Anxiety Disorder Assignment Paper, an increase in the dosage meant more efficacy rate for SSRIs when addressing anxiety symptoms. This meant that the previously adopted measure to increase the dosage would result in a more positive address to the symptoms recorded in the first phase.
This second option was chosen based on the patient’s previous record and response to the first treatment. The option to introduce another medication was ignored as it would result in possible drug-related side effects. Nevertheless, a higher SSRI dosage is likely to cause severe side effects. The need to change the drug was uncalled, as the attending physician had noted a partially positive response to the first treatment. Garakani et al. (2020) identified that Zoloft-related side effects include insomnia, and dryness in the mouth, which the patient did not experience. More so, it showed the medicine was doing well with him. The focus was to decrease both the HAM-A score and anxiety, hence the expectation for the client to follow the plan or the current treatment to reduce symptoms. The hope with this treatment is to continue improving the client’s quality of life by addressing anxiety symptoms hindering him from achieving the required milestone. Regarding ethical concerns, the need for observing ethical aspects is due to fear of exacerbation symptoms, patients dropping out of treatment plans, safety concerns, and the blurring boundary between clients and therapists Case Study For Anxiety Disorder Assignment Paper.
Decision # 3
Thirdly, decision number 3 is still aligned or focuses on our previous patient and treating anxiety. The decision is to continue with the previous medication Zoloft and the same dosage of 75mg PO daily. From the previous medication uptake, there was a significant improvement in patient anxiety symptoms. More so, the HAM-A score had changed to a single-digit figure, indicating a decline in his symptoms. With these indicators, it was my belief the Zoloft 75mg daily was to remain at his dosage till the anxiety symptoms were fully remitted. The customer is currently taking a daily dose of 75mg of Zoloft without experiencing any negative side effects, which demonstrates remarkable improvement. My third choice, made after watching the symptoms, is to maintain the current dose and have the full effects of the medication analyzed after 12 weeks. It is unnecessary to increase the dose to 100 mg as the patient has demonstrated a reduction of 61%; nonetheless, doing so might cause adverse effects that make it difficult for the patient to adhere to the prescribed course of action. The third option is anticipated to result in a further reduction in GAD symptoms.
The other treatments were turned down due to possible side effects if the dosage was added to 100mg. More so, it was not possible to introduce a new drug at this point since Zoloft was addressing the patient’s and physician’s needs. The greater dosage of SSRIs is beneficial in reducing anxiety symptoms, claim Jakubovski et al. (2019)Case Study For Anxiety Disorder Assignment Paper. The customer also reported serious side effects after taking 75mg of Zoloft daily for four weeks. So, if the client kept taking the present dosage for the following four weeks, their anxiety symptoms would go completely. Like the second decision, ethical considerations were critical to the care administration. The psychiatrist abides by the ethical principle of confidentiality, which forbids releasing a patient’s health information to third parties without that patient’s permission. Hence, before disclosing the patient’s medical information to other healthcare professionals or family members, the psychiatrist would get the patient’s permission.
Conclusion
The client’s anxiety symptoms would be managed using three therapy options. Starting with Zoloft 50 mg PO daily is the first available choice. Anxiety problems in adults can be effectively treated with Zoloft, an SSRI. The client returned for the clinic appointment in week four, reporting a decrease in the sporadic sense of impending doom and anxiety about work. This indicates that the expectations were met. The only practical course of action for the greatest results and efficient management of the symptoms was to continue taking Zoloft; therefore, other potential therapies were declined. The use of the HAM-A score was crucial for determining the direction and development of the reaction to the chosen medicine. Using Zoloft greatly reduced the client’s anxiety symptoms Case Study For Anxiety Disorder Assignment Paper.
