Discussion response to Keisha C

Hello,

The presented case scenario is of a 26-year old Lebanese female living in a graduate-student housing. Before getting into a conversation with a patient as a healthcare provider, it is essential for the provider to apply their professionalism towards ensuring that a trust is achieved between them and the interviewee as you have stated. It can also be achieved through establishing a good rapport with the patient. I agree with you that promoting the interview with a patient Centred approach is important, especially considering that she is from a Lebanese culture where respect between opposite genders is highly observed (Lebanese Culture Core Concepts, 2023)Interviewing The Patient Discussion Paper. Every interview should start with courtesy to ensure that the patient is comfortable in all aspects. Furthermore, determining any language barriers as you have stated is also essential towards a better understanding. Cultural considerations are also essential in such a scenario. The provider should therefore be concerned of any Lebanese beliefs to ensure that they do not frustrate the patient during the interview. I also agree with you that some quality communication techniques that can be used include asking open ended questions while observing the patient’s nonverbal ques. Furthermore, I agree with you that making use of simple medical terms is also essential. However other communication techniques include asking precise and clear questions to the patient.

Various risk assessment tools that could be utilized in the presented case include suicidal thoughts screening and Partner Violence Screen (PVS) as you have stated. These will ensure the screening of risk factors from all aspects including social life, health history, social history and medical history (Wong et al., 2018). The tools are also essential towards determining intimate partner violence. The questions that should be asked to such patient should be based on their personal details such as name and age, her social life, the reason for visiting the clinic and any medications that she might currently be taking (Ball et al., 2019)Interviewing The Patient Discussion Paper. Asking the questions should ensure a proper capturing of physical health assessment as well as identifying any other health needs. I am glad to have read you post and have gained a lot concerning the vital components of an interview between a healthcare provider and a patient.

Discussion response to Billie L

Hello,

An interview is a critical conversation that occurs between two individuals or parties. Whenever conducting an interview, the interviewer must first observe the interviewee’s general attitude. I agree with you that establishing a rapport with the 14year old patient is the first step of an interview. A rapport is essential towards ensuring a safe environment that is free from judgements as you have stated. Various techniques of effective communication when interviewing the patient include active listening, use of open-ended questions, applying non-directive approach whenever talking to the patient and observing their sensitivity concerning their cultural background as you have stated (Baby, Gale & Swain, 2018)Interviewing The Patient Discussion Paper. Furthermore, an effective communication during an interview should involve short and clear questions, and ensuring that the questions are not too much to attract boredom of the patient.

One of the best screening tools for assessing mental health problems among children between age 3 to 16 years is the Strengths and Difficulties Questionnaire (SDQ). It is a tool that can be filled by either the child themselves, parents or their teachers, to help in assessing a child mental health (Hoosen et al., 2018). The screening tool would therefore be applicable in the case of the 14-year old presented in the above scenario. I agree with you that the questionnaire involves 25 different questions that help to capture all the possible mental health symptoms of the patient. The target questions should address the daily patient’s personal behaviors. They should therefore range from his sleeping patterns, stress management, their nature of biracial identity, people helping them in difficulties among other daily life challenges (Ball et al., 2019)Interviewing The Patient Discussion Paper. Asking questions based on these aspects is essential since they mostly trigger mental health problems. I therefore commend your five questions of consideration since they address these issues. I am glad to have read your paper and have gained a lot concerning mental health assessment in adolescent individuals.

14-year-old biracial male living with his grandmother in a high-density public housing complex
Post a summary of the interview and a description of the communication techniques you would use with your assigned patient. Explain why you would use these techniques. Identify the risk assessment instrument you selected and justify why it would be applicable to the selected patient. Provide at least five targeted questions you would ask the patient.

Initial Discussion Post
This discussion covers an interview with a 14-year-old male patient living with his grandmother in a high-density public housing complex. It covers the summary of the interview conducted to understand the patients, the communication techniques used, the risk assessment tool selected for the case, and the targeted questions.
Interview summary
During an interview with my assigned patient, I would first focus on establishing a rapport with him. This would encompass creating an environment that is safe and free from judgments. Exploring his experiences, feelings, and challenges related to living arrangements, biracial identity, and other potential risks, he might be facing would be very important (Vernon et al., 2021). My main aim would be to gather information to be able to assess his resilience, mental health, and any potential protective factors.
Communication Techniques
• Open-ended questions-the main focus here would be undertaking the patient well by encouraging him to share more about his place of residence, how he relates with people, and express his feelings and thoughts in detail (Vernon et al., 2021).
• Active listening- allowing him to express himself without interruption, listening attentively, and providing both verbal and non-verbal cues to him (Finney et al., 2020).
• Use a non-directive approach by allowing him to take the lead in the conversation so that he is free to explain important topics to him while guiding him to the relevant discussion gently (Vernon et al., 2021).
• First, Observe Cultural sensitivity by recognizing and respecting his biracial identity.

Risk Assessment Instrument
I selected the Strengths and Difficulties Questionnaire (SDQ) tool because of its known use in assessing both behavioral and emotional problems in children and adolescents (Ball et al., 2019). The questionnaire is made up of a total of 25 questions that cover different domains, such as peer relationship issues, inattention, prosocial behaviors, and emotional symptoms. It can help identify the risks he might face (Ball et al., 2019).
Targeted Questions
1. How do you handle difficult situations or stress in life?
2. How is your sleeping arrangement with your grandmother, and how do you feel living in a place with many people?
3. If you need any help, are there people who can come through except your grandma?
4. What can you say about your biracial identity, and how do you feel about it?
5. Do you face any specific challenges daily?

Week 1 Discussion Post
Health Assessment

“Assignment: Post a summary of the interview and a description of the communication techniques you would use with your assigned patient. Explain why you would use these techniques. Identify the risk assessment instrument you selected and justify why it would be applicable to the selected patient. Provide at least five targeted questions you would ask the patient. Scenario: 26-year-old Lebanese female living in graduate-student housing:”

Interview and Communication Techniques
As a healthcare provider, building trusting relationships with patients is essential to promote a healthy provider-patient bond. Patients need to feel comfortable with healthcare providers in order, to be honest and forthcoming in regard to their health. In this scenario, the healthcare provider is assessing a 26-year-old female patient; she is Lebanese and lives in graduate-student housing. I would ensure the interview process promoted and established trust by providing patient-centered care (Ball et al., 2019). Patient-centered care will ensure respect is given to the patient and a thorough response to the patient’s expectations, including needs and wants (Ball et al., 2019). The interview would begin with trying to establish a connection and offering courtesy, such as knocking before entering the room and ensuring the patient is comfortable, as well as identifying any language or communication barriers (Ball et al., 2019). If a language barrier is present, a translator or translation device will be used. Comfort will be evaluated to ensure the patient is physically and emotionally comfortable ( Ball et al., 2019). Cultural considerations would be initialed prior to entering the room. I politely enter the room and also see if anyone else is with the patient or if she is alone, if others are present with the patient it will be important to see how they are connected with the patient (Ball et al., 2019). I would ask the patient what brought her into the clinic today and proceed with more specific questions depending on her answer. I could utilize open-ended questions as well as observe for non-verbal responses and ques the patient may exhibit during the interview. Important aspects of the interview would include ensuring the patient’s information is correct such as name and date of birth, why she is at the clinic, her previous medical problems, allergies, and current medications, and afterward an examination and review of systems (Ball et al., 2019)
As the patient’s healthcare provider, communication techniques would include the use of simple questions and responses. Medical terminology and the use of hard-to-understand medical acronyms would be avoided (Ball et al., 2019). Communication would be patient-centered and sensitive to cultural considerations. Lebanese culture exhibits some Arabic considerations, and men and women may not physically interact with individuals of the opposite gender; however, a smile is considered polite (Culture Atlas., 2023). Eye contact would be a good communication technique and would also be culturally appropriate (Ball et al., 2019). Good eye contact can build trust. Patient-centered communication will promote the development of a positive relationship between me, the provider, and the patient.
Risk Assessment
Risk factors identification is crucial to ensuring patients are cared for and receiving treatment for all aspects of their health, including physical, mental, and emotional. Risk factors can be identified by the patient’s health history, family history, social history, and even cultural and socioeconomic variables (Sullivan, D., 2019). All patients should be screened for suicidal ideations or risks at each visit to a healthcare facility. This patient would be screened for this; however, another important screening for her would be the Partner Violence Screen (PVS) (Ball et al., 2019). This screening is vitally important due to patients who experience intimate partner violence are largely women and children (Ball et al., 2019). She will be asked brief questions regarding previous abuse, if she currently feels safe, and if has previously felt unsafe in a relationship (Ball et al., 2019). Her responses will determine where the interview and assessment process will go.
Five Targeted Questions:
As the healthcare provider ensuring appropriate questions is essential for a thorough health assessment. Since I will be utilizing a patient-centered approach five targeted questions would include:
1) Hi, I am your healthcare provider, Keisha, can I ask your name and how you would prefer to be addressed today?
-Once we have had this conversation and addressed any language barriers, I will proceed.
2) How can I make you comfortable or do you have any cultural or religious considerations I need to honor?
-I would amend my care based on her response.
3) What brings you in today?
-I would actively listen and ask any pertinent questions pertaining to her answer.
4) Do you mind if I complete a physical exam and ask about your present and past medical history, including social and family history?
-If permission is given, I would complete my exam and Questionnaire.
5) Do you take any medications or do you have any allergies?
-This is extremely important if medications are needed to be given.
These questions can open the door for a more thorough exam and positive patient experience. By allowing the patient to lead the conversation and give permission prior to any examination she has control and can feel more secure.
