Which of the Culture and Caring Theories Most Resonates with You for Your Practice as an NP

Among the various cultural and caring theories in nursing, the Purnell Model for Cultural Competence most strongly resonates with my future practice as a nurse practitioner (NP). This model underscores the importance of understanding and respecting cultural diversity in healthcare delivery. It stresses that effective treatment should align with patients’ cultural beliefs, values, and traditions to support health outcomes consistent with their worldview (Purnell, 2018).

Growing up in the United States as the child of Indian immigrants, I observed firsthand how cultural adaptation can vary across family members. While my brother and I adjusted relatively quickly, my parents faced more significant challenges navigating language barriers, traditions, and differing healthcare expectations. These experiences reinforced the idea that cultural sensitivity extends beyond patient care—it influences relationships with colleagues, families, and communities within the healthcare environment.

The Purnell Model integrates seamlessly with the nursing paradigm by addressing the patient, family, community, and society. It outlines 12 cultural domains, including communication, family dynamics, spirituality, nutrition, and healthcare practices, which help nurses avoid stereotypes and deliver individualized care (Purnell, 2018). This holistic perspective supports the delivery of genuinely patient-centered care.

Integration of the Theory with the Nursing Paradigm

The nursing paradigm consists of four fundamental elements: person, health, environment, and nursing. The Purnell Model aligns with each as shown below:

How the Theory Meets CLAS Standards to Advance Health Equity

The National Standards for Culturally and Linguistically Appropriate Services (CLAS) advocate for fairness, clarity, and respect in healthcare. The Purnell Model supports these principles by offering structured strategies to promote health equity (Narayan, 2017).

What Parts of the Theory Do You Identify With?

I personally identify most with the communication and family roles domains of the Purnell Model. Many patients depend on family members in healthcare decision-making, and understanding these dynamics helps in creating more effective, patient-centered care plans. Additionally, recognizing diverse communication styles—such as variations in eye contact or nonverbal cues—reduces misinterpretation and builds stronger nurse-patient trust.

The model’s emphasis on lifelong learning also resonates deeply with me. Cultural competence is an evolving skill that grows with each patient interaction. As an NP, I view each encounter as an opportunity to refine my cultural understanding and enhance my approach to holistic care.

This week, I achieved the objectives by critically analyzing cultural and caring theories, focusing on the Purnell Model and its relevance to advanced nursing practice. For example, I explored how cultural competence shapes individualized care plans—such as accommodating dietary customs or religious observances. Integrating these factors not only enhances patient satisfaction but also fosters trust, which is fundamental to effective therapeutic relationships.

What Personal Values Were Challenged This Week?

Throughout this week’s learning, I became aware that despite valuing inclusivity, unconscious bias can still influence my perceptions. This realization challenged me to reflect on subtle assumptions that might affect my approach to care. Recognizing this has motivated me to practice greater humility, openness, and curiosity when engaging with culturally diverse patients.

What Values Can You Reaffirm or Reconsider After This Learning?

I reaffirm the core nursing value of caring, which extends beyond the physical realm to include cultural, emotional, and social well-being. Respect for diversity remains a guiding principle in my practice since each patient’s cultural background significantly influences their health behaviors and expectations.

Additionally, I have reconsidered the importance of patient autonomy and shared decision-making. Integrating cultural perspectives into clinical discussions encourages collaboration and ensures care plans reflect patients’ individual preferences and values.

Do You Think if Cultural Theories Were Used or Taught More, Would This Be Helpful?

Yes, the increased teaching and application of cultural theories would be profoundly beneficial in both nursing education and practice. These frameworks provide healthcare professionals with the tools to effectively manage cultural diversity and improve care delivery. The advantages include:

• Enhanced communication between providers and patients

• Stronger trust and rapport within healthcare relationships

• Higher quality of care and improved patient outcomes

• Reduction in health disparities through culturally respectful practices

When nurses actively apply cultural models, they become better equipped to provide equitable and inclusive care across diverse populations.

Example of Another Relevant Theory

In addition to the Purnell Model, I find the Person-Centered Nursing (PCN) framework by McCormack and McCance (2006) particularly valuable. This model highlights dignity, respect, and individualization in nursing care. Unlike theories that focus primarily on diseases, PCN emphasizes treating patients as whole individuals—taking into account their personal values, preferences, and life experiences.
