- Compare and Contrast past and present major influences on the changing health care environment that impacted current professional nursing practice.
Nursing practice has, with time, included consideration for factors that affect the health of individuals and communities. Attention to these factors is included in the scope of nursing practice. Nonetheless, for the last 60 years, the major influences have been changes in healthcare delivery, funding for public health, credentialing requirements, novel and unfamiliar ethical dilemmas, and professional associations. First, there has been a significant change in healthcare delivery as facilitated by advanced information technology. Care has shifted from in-person to remote care delivery through telemedicine and telehealth, creating a need for nurses to understand information technologies. Second, there has been a change in models applied to funding for public health as presented in payment and reimbursement models. The models have shifted towards cost management, care quality, and patient safety, and professional nursing practice has similarly changed to accommodate payment and reimbursement models. Third, there has been a change in credentialing requirements as nursing specializations have developed along with credentialing requirements for different levels of practice and roles (Taylor et al., 2022). Fourth, there have been novel and unfamiliar ethical dilemmas that require current nursing practice to reexamine and confront conflict between legal and ethical issues. An example is current nursing practice confronting the ethics of euthanasia. Fifth, professional associations have developed to represent the interests of nurses, advocate for their needs, and help nursing practice advance, such as advocating for nurses’ education and removal of practice restrictions (Murray, 2022). Describe specific behaviors you have employed to incorporate the art and science of nursing into your practice. Explain how these behaviors have assisted in your professional role development.
Caring, compassion, effective communication and holistic care are specific behaviors that incorporate the art and science of nursing into practice. First, caring behavior involves speaking to the patient in understandable terms, talking to the patient, touching the patient when in need of comfort, putting the patient first, irrespective of other issues, and listening to the patient. Caring assists in professional role development by creating an optimal patient experience that addresses every aspect of the patient’s encounter, including physical comfort and spiritual, emotional, and educational needs. Second, compassion behavior involves shifting from simply providing competent nursing care that includes the required knowledge and skills to treat the patient to engage in deeds and actions that address the emotional aspects of the nurse-patient relationship. Compassion provides patients with emotional support that strengthens the will to survive and lessens depression. This includes helping relieve concerns, explaining and answering questions, using a positive voice and body language, providing emotional support, being an active listener, getting to know the patient better, and being empathetic (Zerwekh & Garneau, 2022). Third, effective communication involves approaching the patient’s interactions to understand the patient’s opinions, experiences, and concerns. This includes applying patient teach-back techniques, active listening, and verbal and non-verbal communication skills. It encompasses written communication, non-verbal communication, presentation skills, patient education, cultural awareness, personal connections, trust, verbal communication, and active listening. This is important for collaborating on teams and delivering patient-centered care. Fourth, holistic care that looks at the patient beyond the condition, illness or personal health goal. It looks at the aspects that determine the health and care journey, including personal and social circumstances, cognitive health and behavioral elements, spiritual beliefs, and mental and physical health. A nurse should be able to see the patient as a whole person and apply professional knowledge and judgement to meet the patient’s needs, drawing on expertise from other fields as appropriate. This helps professional role development by justifying continuous education to acquire professional knowledge and skills that facilitate continuity of effective, kind, safe care (Huber & Joseph, 2022).
